About The Position

This position is for a Nurse in Crisis Receiving Stabilization Services and Psychiatric Urgent Care located in Peru, Indiana. 4C Health is a trusted, non-profit, Certified Community Behavioral Health Clinic (CCBHC) that has been proudly serving the communities of North Central Indiana for over 50 years. They began as the designated community mental health center for Cass, Miami, Fulton, and Pulaski Counties and have expanded services to Howard, Tipton, and White Counties. Requirements

  • Licensed as an RN in the state of Indiana, current and valid.
  • Willing to submit to a pre-employment drug screen.
  • Willing to submit to comprehensive background checks, including but not limited to criminal and child protective services checks.

Nice To Haves

  • Experience in psychiatric nursing preferred.

Responsibilities

  • Complete nursing assessments and client assessments that include intake, evaluating a client’s ability to meet the admission criteria for that particular department, identify presenting problem(s), history, medical information, family involvement, psychosocial/psychosexual information, and other pertinent information to form an appropriate clinical impression including substance use/abuse and abuse/neglect.
  • Document timely, accurately and in accordance with Center policies utilizing current systems and practices.
  • Document accurate information, which reflects client progress based on the appropriate therapeutic modality.
  • Request documentation from other facilities, as needed.
  • Work with insurance companies to provide the required information and complete insurance documentation.
  • Record delivery of all directed medications and treatments per treatment plan and policy.
  • Deliver medications and other medical treatment as directed by physician’s orders.
  • Deliver therapeutic services commensurate with skill level.
  • Supervise subordinate personnel as assigned (i.e. client care specialists and medical assistants).
  • Transcribe orders and call medications in to pharmacy.
  • Schedule appointments and assist clients in obtaining appointments.
  • Demonstrate an understanding of medication protocols and able to answer basic questions about medication.
  • Complete referral paperwork as needed.
  • Obtain requested information from other physicians/agencies.
  • Demonstrate the ability to accurately assess and document a client’s medical status; including but not limited to taking vital signs, height, weight, BMI, and suicidal ideations reporting any concerns to the appropriate personnel.
  • Obtain client consents for releasing information.
  • Demonstrate an understanding of basic concepts regarding practice of Licensed Independent Practitioner (LIP).
  • Teach common activities of daily living as needed.
  • Facilitate and provide education to the public, staff, groups and individualized education to clients and their families as indicated.
  • Teach, monitor, and evaluate the competence of client care specialists and medical assistants in medical equipment, groups, and nursing care of clients.
  • Provide emergency services as needed, either by phone or face-to-face.
  • Participate in an on-call rotation as required by the department manager per Center policy if necessary.
  • Abide by all Center policies and procedures.
  • Maintain positive working relations and rapport with the communities served.
