About The Position

Collage Home & Community is seeking an experienced Registered Nurse (RN) to serve as a Nurse Supervisor supporting our Texas Licensed Home Health Services (LHHS) operations. This role will initially begin as PRN and is expected to ramp quickly to a full-time position as new clients are onboarded and service volume grows. This is an excellent opportunity for a clinically strong RN who wants to help build and scale a high-quality, compliance-driven home health program focused on medically complex patients. This position is intentionally structured to allow the right candidate to grow into a long- term leadership role as the program expands.

Requirements

  • Current, unrestricted Registered Nurse (RN) license in the State of Texas
  • Minimum of 2 years of nursing experience
  • Strong knowledge of clinical supervision and care planning
  • Current CPR certification
  • Valid Texas driver’s license and reliable transportation

Nice To Haves

  • Home health, community-based, rehab, or complex care experience preferred
  • Familiarity with Texas home health regulations preferred

Responsibilities

  • Serve as the supervising RN for nursing and aide services provided under the agency’s Texas LHHS license
  • Provide clinical supervision of RNs, LVNs, and Certified Nurse Assistants (CNAs)
  • Ensure CNA services are appropriately assigned, delegated, and supervised by nursing
  • Conduct or oversee initial and ongoing patient assessments
  • Develop, review, and update physician-ordered plans of care
  • Monitor patient progress and respond to changes in condition
  • Ensure services are delivered in compliance with Texas Health & Safety Code Chapter 142 and 26 TAC Chapter 558
  • Maintain accurate, timely, and complete clinical documentation
  • Participate in audits, surveys, and quality improvement activities
  • Partner with intake, operations, and leadership to support safe onboarding of new clients
  • Help establish scalable clinical workflows as census grows
  • Serve as a clinical resource to staff and leadership
