About The Position

In collaboration with senior leadership, oversees hospice program development including, but not limited to, interview and hiring of staff, staff education, data management, and implementation of appropriate clinical practices in the hospice program. Determine staffing needs according to census requirements and with consideration for utilization of personnel at their maximum potential. Oversee compliance with personnel policies. Reviews performance and performs evaluations of all direct reports, and when necessary, determines and administers corrective action. Monitor day-to-day compliance with regulatory and reimbursement requirements as defined by various government agencies, and generally directs the staff in the care of patients on the unit. Maintain compliance with all Joint Commission, state and federal regulations. Assists with the orientation, preceptorship and ongoing education of all McCarthy Care Center staff. Collaborates with other departments to develop, maintain and monitor policies and standards related to hospice activities. Determines patient eligibility for General Inpatient level of care within the program guidelines established in collaboration with hospice medical director, interdisciplinary team members and hospice intake department. Performs the admission process to include all signed forms, physician orders and CTI’s completed, establish patient plan of care including medication reconciliations. Initiates the GIP (General Inpatient) Plan of Care based on the patient and family/caregiver needs reflecting assessment data obtained to determine the nursing diagnosis and establish goals for care. Includes both the patient and family/caregiver in the care planning process. Re-evaluates this Plan of Care regularly and revising as needed to reflect the patient and family/caregiver care needs. Manages patient symptom management; administers medications and treatments as prescribed by the physician. Attends the Interdisciplinary Group Team meetings regularly. Participates in Hospice sponsored in-service training and mandatory trainings. Ensures that equipment, medication and other necessary items and services are available to the patients in the McCarthy Hospice Care Center. Provide clinical expertise and serves as a resource to staff and the community. Participates in Agency Quality Improvement program. Consistently provides service excellence to all patients, family members, visitors, volunteers and co-workers in a manner that reflects VNA’s commitment to CARES: compassion, accountability, respect, excellence and service. Other duties as assigned.

Requirements

  • Registered Nurse currently licensed in the Commonwealth of Massachusetts.
  • BSN, Master’s degree in Nursing preferred.
  • Minimum of two to four years of supervisory experience in a home health agency, hospice or related health program.
  • Ability in program administration.
  • Excellent oral and written communication skills.
  • Ability to utilize available hardware and software.
