About The Position

The Nurse Supervisor-LVN position at Santa Cruz Post Acute involves regular communication with residents and their families regarding Plans of Care, physical therapy (PT), occupational therapy (OT), and other treatment protocols. The role requires participation in morning meetings to facilitate team communication, answering residents' questions about their care and treatment plans, and monitoring and adjusting residents' statuses based on their changing needs. The Nurse Supervisor is responsible for organizing and prioritizing daily work, completing necessary documentation, performing utilization review activities, and coordinating care with various stakeholders including residents, care providers, financial services, and third-party payers. The position also oversees admissions and discharge activities and coordinates referrals to and from the facility. The ideal candidate must possess excellent communication skills, time management abilities, and the capacity to work independently while exercising sound judgment.

Requirements

  • Bachelor's Degree in Nursing or Social Work.
  • Registered Nurse (RN) license preferred; Licensed (LVN or LPN) nurse acceptable.
  • Two (2) years of clinical nursing experience preferred.
  • Knowledge of Medicare, Medi-Cal, and Medicaid programs and benefits.
  • Ability to read technical procedures and comprehend policy manuals.
  • Ability to effectively present information and respond to questions.
  • Mathematical skills to apply concepts such as fractions, percentages, ratios, and proportions.
  • Ability to solve practical problems and interpret various instructions.

Nice To Haves

  • Certificate as a certified Case Manager (CCM) is a plus.
  • Experience in a supervisory role.

Responsibilities

  • Communicate regularly with residents and their families about Plans of Care, PT, OT, and other treatment protocols.
  • Attend and participate in morning meetings to facilitate team communication.
  • Answer residents' questions about their care, treatment plans, and illness progression.
  • Monitor and adjust residents' statuses based on changing needs and conditions.
  • Organize and prioritize daily work by assessing new, current, and discharging residents' needs.
  • Complete documentation as required.
  • Perform utilization review activities to provide appropriate, timely, and cost-effective care.
  • Coordinate care with residents, care providers, financial services, and third-party payers.
  • Oversee all admissions and discharge activities.
  • Coordinate referrals both to and from the facility.
  • Maintain professional, respectful, and honest interactions with residents, families, and staff.
  • Demonstrate excellent time management skills in a high-volume environment.
