About The Position

Nurse Specialist are responsible to prioritize/manage their time/resources as applicable to the needs of the outpatient and/or inpatient settings. They Coordinate communication between Physicians and Medical staff members, other outpatient and inpatient departments/staff, patient and family. They Gather clinical information, results of tests, discharge plans, and changes in patient condition to facilitate patient and staff communication with Physicians. Department: Urology Clinic Hours: Full-time, 40 hours/week Required: ADN, IL-RN; BLS & ACLS within 90 days of hire Pay: Based on experience, starting at $33.73

Responsibilities

• Assists in outpatient clinic by rooming patients, covering the Nurse Desktop and performing other duties typically assigned.
• Assists physician with patient exams/ treatments/procedures during inpatient rounds as needed.
• Coordinates communications with referring physicians, specialists, patients, families and hospital departments/staff to facilitate timeliness and thoroughness of information shared.
• Documents responses to patient history/screening questions for use by providers during assessments and examinations. Computer documentation proficiency to perform this task on both the outpatient and inpatient setting.
• Monitors patients with chronic conditions and ensures appropriate education and expectations with the next level of care to support health maintenance and reduce readmission risks.
• Prioritizes assigned physician workload related to inpatients, observation patients and patients presenting to the Emergency Department.
• Relays information between provider(s) and client(s), which may include but is not limited to communications with patients, families, pharmacies, nursing homes, consultancy office staff, insurers, and facilities. At their physician's instruction, communicates provider advice, instructions, orders and results, as applicable, including accurate documentation.
• Responds to all department-specific emergencies in the Heart Center and Cath Lab to assist with performance of appropriate ACLS treatment.
• Reviews patient record for complete documentation and compliance with Core Measures and Quality Metrics and other regulatory and quality requirements.
• Rounds on assigned inpatients to assess response to treatment, results of tests, patient/family concerns and questions and readiness for discharge. Communicates information to their assigned physician and documents information and/or orders as requested.
• Supports marketing of program with internal and external customers and primary care providers.
