About The Position

The Fogarty Center is seeking a dedicated Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join their team as a Residential Health Care Coordinator. This full-time position involves coordinating nursing care for individuals in community group homes, ensuring high-quality healthcare services, and fostering a supportive environment for both staff and participants. The role emphasizes collaboration, education, and proactive health management for approximately 75 participants across Rhode Island.

Requirements

• Current Registered Nurse License (required)
• Experience in the I/DD field
• Confidence in decision-making and sound judgment
• Self-sufficient and resourceful with the ability to build networks and relationships
• High work ethic, detail-oriented, and committed to follow-through
• Valid driver's license and access to an insured vehicle (required)
• Clear BCI (required)

Responsibilities

• Coordinate nursing care for 5-6 community group homes.
• Manage individualized healthcare plans with participants' providers, including attending appointments as needed.
• Conduct regular visits to each home to review documentation and attend staff meetings.
• Educate direct care staff on best practices and diagnosis-specific material.
• Support and encourage direct care staff in medication administration and core competencies.
• Foster a positive interprofessional team environment through effective communication with staff, clients, and families.
• Monitor medications for adverse reactions and report effects to family and/or physician.
• Oversee medical issues and alert supervisors of changes in client health.
• Implement innovative teaching opportunities for direct support staff.
• Be available by phone after hours for medical triage as needed.
