About The Position

The Nurse Preceptor at Oak Street Health is responsible for educating and empowering Center Nurses to deliver high-quality patient care. This role involves onboarding new hires, supporting skill-building for existing nurses, and collaborating with various team members to ensure effective training and care delivery. The Nurse Preceptor will also provide float RN coverage and assist in driving quality care improvements within the region.

Requirements

• Active Registered Nurse (RN) Licensure in good standing with the applicable state.
• 1-year experience as a Nurse at Oak Street Health.
• BLS Certification.
• Electronic Medical Record (EMR) experience.
• Ability to maintain patient confidentiality.
• Strong communication and assessment skills.
• Willingness to travel to all centers regionally.

Nice To Haves

• 3+ years of healthcare experience working as an RN.
• Ability to collaborate with an interdisciplinary care team.
• Excellent computer skills for data interpretation.
• Previous experience in a clinic setting.
• Fluency in Spanish, Polish, Russian, or other languages spoken by the communities served.

Responsibilities

• Educate and empower new Oaky Nurses through onboarding and training.
• Create a welcoming environment for new hires and support their integration into the team.
• Demonstrate clinical knowledge and excellence in nursing education.
• Support remediation and performance coaching for existing Oaky nurses.
• Collaborate with Directors of Clinical Support and center-level leaders to identify precepting needs.
• Provide float RN coverage to support center-level needs as required.
• Conduct chart audits and provide feedback for quality care improvements.
• Participate in care team meetings to discuss patient care and clinic operations.
