About The Position
Gentiva Hospice is seeking a Hospice Nurse Practitioner to join our team. In this role, you will report directly to the Executive Director or Administrator and will be assigned to a location to perform Face-to-Face (F2F) encounters. Your primary responsibility will be to gather clinical information to support the hospice diagnosis and prognosis, which will then be communicated to the hospice physician responsible for certifying or recertifying the patient's hospice eligibility. The scope of practice for the hospice NP is limited to those tasks necessary to complete hospice F2F encounter visits.
