About The Position
ComplexCare Solutions (CCS) specializes in providing high-value in-home health assessments for health plans across the nation. Our mission is to enhance member engagement by evaluating current health statuses, identifying gaps in care, assessing potential health risks, and exploring care management opportunities. We focus on driving better clinical outcomes through personalized care. This position offers Nurse Practitioners (NPs) and Physician Assistants (PAs) the opportunity to work flexibly, with competitive pay and a high degree of autonomy. CCS is an ideal fit for healthcare professionals who are passionate about supporting their communities, educating members about their health, and helping them remain in their homes rather than being hospitalized. As a CCS provider, you will conduct in-home assessments for Medicare, Medicaid, and Commercial health plan members. These assessments are comprehensive and typically include a complete review of medications, a review of systems, a physical examination, and discussions about medical concerns affecting the member. Each visit is allotted up to an hour, allowing for enhanced one-on-one interaction with members, which helps them gain a better understanding of their health and prepares them for future discussions with their primary care providers. CCS providers are true healthcare heroes, connecting with members in the comfort of their homes and making a significant impact on their health and well-being.
