Requirements

• General Post-Acute/SNF knowledge or experience preferred.
• Familiarity with EMR systems and charting practices.

Nice To Haves

• Experience in wound care or other specialized training.

Responsibilities

• Rounding and managing the clinical care of the population at the assigned SNF.
• Prescribing medications and treatments for residents at SNFs.
• Providing comprehensive primary care services to patients residing in SNFs.
• Timely documentation of all patient care encounters.
• Available for phone/secure messaging consultations with facility nursing staff.
