Requirements

  • For Nurse Practitioners: Master’s or Doctorate degree in Nursing from an accredited program.
  • For Nurse Practitioners: Active and unrestricted RN and NP license in the state of practice.
  • For Nurse Practitioners: National board certification as an NP (e.g., AANP or ANCC).
  • For Nurse Practitioners: DEA License required or ability to obtain.
  • For Physician Assistants: Master’s degree from an ARC-PA accredited Physician Assistant program.
  • For Physician Assistants: Active and unrestricted PA license in the state of practice.
  • For Physician Assistants: Current NCCPA certification (National Commission on Certification of Physician Assistants).
  • For Physician Assistants: DEA License required or ability to obtain.
  • Current BLS/ACLS certification (as required by facility).
  • Strong clinical assessment and decision-making skills.
  • Excellent communication and documentation skills.
  • Ability to work independently and collaboratively within an interdisciplinary team.

Responsibilities

  • Provider is responsible for rounding and managing the clinical care of the population at the assigned SNF.
  • Provider is responsible for rounding daily and prescribing medications and treatments for residents residing at SNFs.
  • Provider will provide high quality, patient-centered, comprehensive primary care services to patients residing in SNFs.
  • Provider is responsible for timely documentation of all patient care/encounters.
  • The provider will be available for phone/secure messaging consultations with facility nursing staff.
  • Provider documentation is completed in Theoria’s proprietary electronic health record (EHR) system, ChartEasy™.
  • Provider is expected to be on-site on and on time for their in-person scheduled shifts.
  • Reliable transportation required.
