About The Position
Theoria Medical is seeking a dedicated provider to join our team, responsible for rounding and managing the clinical care of patients at skilled nursing facilities (SNFs). This full-time, exempt position offers a competitive salary of up to $165,000 annually, along with a sign-on bonus to welcome you to the team. The role emphasizes high-quality, patient-centered care, with a focus on establishing meaningful connections with patients and staff. The position requires daily rounding, prescribing medications, and timely documentation of patient encounters using our proprietary electronic health record system, ChartEasy™. The provider will also be available for consultations with facility nursing staff via phone or secure messaging. This is an in-person role with a structured shift from 7 AM to 1 PM, Monday through Friday, allowing for a balanced work-life experience.
