About The Position

The Nurse Practitioner position at ComplexCare Solutions (CCS) involves conducting in-home health assessments for Medicare, Medicaid, and Commercial health plan members. Requirements

• Active un-encumbered nursing license.
• Nurse Practitioner Certification from ANCC or AANP in Family, Adult, Gerontology, or Emergency Medicine.
• Board Certified through AANP or ANCC.
• Current CPR certification.
• Compliance with healthcare personnel vaccination and testing requirements.

Nice To Haves

• Home health experience.
• Understanding of Medicare, Medicaid, and health plan benefit structures.
• Bilingual or multilingual skills.

Responsibilities

• Conduct in-home health assessments for members.
• Perform comprehensive evaluations including medication reviews, physical examinations, and discussions of medical concerns.
• Engage with members to educate and empower them about their health.
• Document findings and provide talking points for members to discuss with their primary care providers.
• Schedule visits flexibly between 8am-8pm, Monday to Sunday.
