About The Position
Theoria Medical is seeking a part-time provider to join our innovative healthcare team. This position involves managing clinical care for residents at skilled nursing facilities (SNFs). The provider will be responsible for daily rounding, prescribing medications, and delivering high-quality, patient-centered primary care services. Documentation of patient encounters is essential and will be conducted using our proprietary electronic health record system, ChartEasy™, as well as other commonly used EHR systems. The role requires timely attendance and reliable transportation, as it is an in-person position with shifts scheduled from 7AM to 1PM local time, Monday through Friday.
