The Nurse Practitioner position at Comprehensive Rehab Consultants focuses on providing specialized Physical Medicine and Rehabilitation (PM&R) services within skilled nursing facilities (SNFs).   • Provide specialized Physical Medicine and Rehabilitation (PM&R)/Physiatry services as a rehab consultant.
  • Represent CRC in your facility through strong clinical leadership and collaboration with the interdisciplinary care team.
  • Collaborate with PCP teams to help manage conditions common in SNFs such as CVA, progressive neurological conditions, and postoperative orthopedic surgeries.
  • Work alongside PT, OT, and SLP teams to improve patient function and quality of life.
  • Follow therapy patients to track their progress in therapy.
  • Assess and collaborate on the management of barriers to therapy including joint or neuropathic pain, spasticity, neurogenic bowel, and neurogenic bladder.
  • Attend multidisciplinary meetings and engage in culture-building opportunities and ongoing education.
  • Certified Nurse Practitioner (CNP) or Physician Assistant (PA) License in California.
  • Experience in post-acute care preferred.
  • Ability to work autonomously and manage patient care effectively.
  • Strong interpersonal skills and empathy for patients and facility partners.
  • Experience in skilled nursing facilities (SNFs) or post-acute care settings.
  • A passion for serving the geriatric population.
