About The Position

The Hospice Nurse Practitioner at Gentiva Hospice plays a crucial role in providing compassionate care to patients and their families. This position involves performing Face-to-Face (F2F) encounters to gather clinical information necessary for hospice diagnosis and prognosis. The Nurse Practitioner will work closely with the hospice physician to ensure patients meet eligibility requirements for hospice care, contributing to the overall mission of delivering high-quality support and care.

Requirements

• Master's or higher degree in Nursing.
• Completion of a graduate-level nurse practitioner education program accredited by a national accrediting body.
• Current state license as a registered nurse.
• Current state license or certificate as an advanced practice nurse.
• Current national certification as a Nurse Practitioner (ANCC or AANP).
• Must have a collaborative agreement in place.

Responsibilities

• Perform Face-to-Face (F2F) encounters to gather clinical information for hospice diagnosis and prognosis.
• Complete physical assessments for F2F encounters within regulatory time frames established by CMS.
• Review medical treatment plans, diagnostic tests, lab results, and medication lists to provide clinical findings to the physician.
• Make recommendations to the Interdisciplinary Group (IDG) based on findings from the F2F assessment.
• Notify the hospice agency if unable to complete a F2F visit according to schedule.
