About The Position
Liberty Healthcare is offering a full-time Nurse Practitioner position at DHCI, a skilled nursing facility in Smyrna, Delaware. The role involves providing primary care to a caseload of approximately forty older adults and individuals with various physical and cognitive disabilities.
Requirements
- Being a licensed Nurse Practitioner
Responsibilities
- Daily patient evaluation through rounding on units
- Conducting examinations, performing diagnostic tests, providing preventive care, developing and managing treatment plans for acute and chronic diseases, and prescribing medications
- Providing consultation and health education to nursing and ancillary staff
- Collaborating with the facility's Medical Director, nursing team, and ancillary staff
- Actively participating in multidisciplinary treatment team meetings
