The Opportunity As a Primary Care Provider , you’ll deliver comprehensive, patient-centered care to adult patients — emphasizing prevention, health education, and continuity of care. You’ll collaborate with a supportive care team focused on quality outcomes , patient satisfaction , and work–life balance .

Requirements

  • Licensed Nurse Practitioner (NP) or Physician Assistant (PA) in Texas (or eligible)
  • Board certified/board eligible
  • Primary Care or Internal Medicine experience preferred
  • Excellent communication and clinical judgment
  • Passion for patient-centered, value-based care
  • Masters Degree from accredited Nursing Program.
  • 3 + years in Primary Care.
  • Valid State Nurse Practitioner License.
  • Current State D.E.A. Certificate.
  • Certification by one of the Nurse Practitioner certifying bodies (ANCC or AANP) as a Family Nurse Practitioner (FNP) or Adult Gerontology NP.
  • Current BLS and ACLS certification.
  • Any combination of education, training, and experience which demonstrates the ability to perform the duties and responsibilities as described including related work experience.
  • Advanced English is required.
  • Flexible to travel per business needs.
  • The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
  • While performing the duties of this job the employee is regularly required to work standing up, walk, use hands to operate tools and equipment and must be able to exert regularly up to 10 pounds of force, frequently exert 30 pounds of force and occasionally exert 50 pounds of force to constantly perform the essential job functions.
  • The employee will be frequently required to reach with hands and arms, bend, balance, kneel, crouch, crawl, push, and pull.
  • Specific vision abilities required by this job include close vision, distance vision, peripheral vision, depth perception and ability to adjust focus.
  • The employee is subject to environmental conditions, protection from weather conditions but not necessarily from temperature changes.
  • The worker is subject to noise; there may be sufficient noise to cause the worker to shout in order to be heard above ambient noise level.
  • Must be able to travel to multiple locations for work (i.e. travel to attend meetings, events, conferences): Occasionally (1-33%)

Nice To Haves

  • Two years of experience working with a multicultural patient population delivering patient-centric programs that are high quality and cost effective, preferred.
  • Spanish and Creole are preferred.

Responsibilities

  • Perform comprehensive medical assessments and develop personalized treatment plans.
  • Diagnose and manage chronic and acute conditions.
  • Promote wellness through screenings, education, and preventive care.
  • Collaborate closely with physicians, nurses, and care coordinators.
  • Utilize modern electronic health records (EHR) and digital tools to streamline care.
