Requirements

Current NP license, with no sanctions, restrictions, or provisions, in the state of Georgia
At least 1 year of clinical experience
Clinical background in adult, family or geriatric specialties
Experience working with an Electronic health record (EHR)
Effective communication skills are critical
Ease of use with smartphone and email are required
Reliable transportation
Ability to work independently
Willingness to spend at least 30 minutes to 1.5 hours with a patient in their homes, which may or may not be in the presence of pets, and/or family members that are tobacco users

Nice To Haves

Home care or home visiting experience (preferred)

Responsibilities

Manage the transitional and chronic care needs of homebound patients including:
Assessment and Evaluation of current condition
Take past medical history
Review symptoms
Conduct general physical examinations
Review and reconcile medications
Screen for depression
Screen for alcohol and drug abuse
Identify diagnoses to be used in care management and active medical management for purposes of treatment
Recommend appropriate measures, including medication prescriptions and skilled needs for the patient
Efficiently communicate findings in your assessment to the office / supervising physician
Educate patients on topics such as disease process and prevention, medication, and compliance
