About The Position

In this role, the primary responsibility involves delivering healthcare services in a clinical environment to patients spanning various age groups. The focus is on preventing diseases, maintaining overall health, educating patients, and providing counseling. Additionally, collaboration with physicians and local healthcare facilities is essential to ensure service delivery aligns with established guidelines and falls within the defined scope of practice.

Requirements

  • Master's or Doctoral degree in Nursing (MSN or DNP) from an accredited nurse practitioner program.
  • Active state licensure as a Nurse Practitioner.
  • National certification as a Nurse Practitioner in the specific area of specialization (e.g., family practice, pediatrics, geriatrics).
  • Strong clinical and diagnostic skills.
  • Excellent communication and interpersonal skills.
  • Compassionate and empathetic approach to patient care.
  • Possess ability to adapt to challenges on a day to day basis that accompany working in a rural health clinic
  • Must be self-directed, minimal constant oversight, able to follow instructions from Leadership.
  • Must pass background check and pre-employment drug screen.

Responsibilities

  • Patient Assessment: Conduct thorough physical assessments of patients, including medical history, symptoms, and other relevant information. Perform comprehensive health examinations to identify health problems and formulate accurate diagnoses.
  • Diagnosis and Treatment: Analyze test results, X-rays, and other diagnostic information to develop appropriate treatment plans. Prescribe medications, treatments, and therapies based on the diagnosis and patient's medical history. Provide counseling and education to patients regarding their health conditions, medications, and preventive measures.
  • Collaborative Care: Collaborate with physicians, nurses, and other healthcare professionals to plan and execute patient care plans. Consult with specialists and refer patients to other healthcare providers as necessary.
  • Chronic Disease Management: Manage and monitor patients with chronic illnesses, adjusting treatment plans as needed to ensure optimal health outcomes. Educate patients on self-management techniques for chronic conditions.
  • Preventive Care: Promote preventive healthcare measures, including vaccinations, screenings, and lifestyle modifications. Conduct health and wellness assessments and provide recommendations for maintaining good health.
  • Record Keeping and Documentation: Maintain accurate and detailed patient records, including medical histories, examination results, treatment plans, and progress notes. Ensure compliance with legal and institutional requirements for record-keeping and documentation.
  • Health Promotion: Participate in community outreach programs and health education initiatives to promote overall community health and well-being. Provide educational materials and resources to patients and their families to enhance health literacy.
  • Continuous Learning: Stay updated with advancements in healthcare, medical technologies, and treatments through continuous education and professional development.
