What will you do: As an Inpatient Palliative Care Nurse Practitioner with Andwell Health Partners, you will provide consultative palliative care to hospitalized patients with serious illness. You will work onsite within our partner hospital, responding to provider orders for palliative care consults, performing initial assessments, and conducting follow-up visits as needed.

Requirements

  • Master’s degree in nursing and national NP certification (AANP, ANCC, or similar).
  • Current Maine APRN license.
  • Palliative care or hospice experience required; inpatient or hospital-based experience strongly preferred.
  • Ability to navigate multiple EMRs efficiently in a fast-paced clinical environment.
  • Strong communication and interpersonal skills with comfort leading complex discussions.
  • Motivation, independence, and excellent clinical judgment.

Nice To Haves

  • ACHPN certification or willingness to obtain within two years.

Responsibilities

  • Complete inpatient palliative care consultations requested by hospitalists, specialists, and inpatient providers.
  • Lead and support family meetings focused on goals of care, code status, symptom management, and transitions, including hospice enrollment when appropriate.
  • Partner closely with inpatient case managers, hospitalists, nursing staff, and the Andwell palliative care team.
  • Provide expert symptom management grounded in evidence-based practice.
  • Document care in multiple EMRs and ensure timely, accurate consult notes.
  • Participate in interdisciplinary discussions and daily care coordination with hospital teams.
  • Serve as an educational resource for hospital staff on palliative care principles and communication best practices.
  • Contribute to quality improvement, program development, and consistent delivery of high-quality palliative care services.
