Role Overview: Nurse Practitioner - Full Time - Hampton, VA We are seeking a motivated and compassionate Nurse Practitioner to join our Hampton, VA team on a full‑time basis. This role focuses on providing high‑quality primary and acute care within skilled nursing and long‑term care facilities. The successful clinician will work closely with facility staff to support resident‑centered outcomes and help reduce avoidable hospital admissions.

Requirements

  • Master’s degree or post‑graduate Nurse Practitioner qualification from an accredited nursing program
  • Active and unrestricted Nurse Practitioner license in Virginia and current DEA registration as required
  • Proficiency with Electronic Medical Record systems and accurate clinical documentation
  • Excellent clinical judgement, strong communication skills and the ability to work collaboratively within interdisciplinary teams
  • Current Basic Life Support (BLS) certification required
  • Compassionate, motivated and professional approach to resident‑centered care

Nice To Haves

  • Experience in post‑acute, long‑term care, gerontology or related settings preferred
  • additional certifications (e.g. ACLS) desirable depending on facility needs

Responsibilities

  • Provide high‑quality primary and acute medical care to residents in post‑acute and long‑term care settings
  • Perform timely on‑site assessments, diagnosis and management of acute clinical changes
  • Manage chronic conditions with a focus on preserving function, optimizing quality of life and reducing avoidable hospital admissions
  • Develop individualized care plans and set measurable goals in collaboration with residents, families and facility staff
  • Order and interpret diagnostic tests, initiate treatments and coordinate specialist referrals as required
  • Provide safe and effective medication management, including reconciliation and deprescribing when appropriate
  • Document patient encounters accurately and promptly in the Electronic Medical Record (EMR)
  • Collaborate with multidisciplinary teams through rounds, care‑planning meetings and family conferences to ensure coordinated, person-centered care
  • Support facility quality and safety initiatives such as infection prevention, falls reduction and medication safety
