About The Position
At EasyHealth, we partner with leading health plans to deliver in-home Annual Wellness Visits (AWVs), bringing preventive care directly to members in their homes. This role is focused on assessment-based care, with no prescribing, no call duties, and no weekend work, allowing the practitioner to concentrate on high-impact patient engagement.
Requirements
- Active Oregon NP/APRN license to conduct visits in home
- Medicare/Medicaid enrollment (or willingness to enroll)
- Board Certification
- Comfort working independently in a mobile care model
- Reliable transportation
Responsibilities
- Conduct in-home health assessments (history, vitals, screenings, labs)
- Identify care gaps and support preventive care initiatives
- Document visits in our EMR and coordinate follow-up care when red flags are identified
- Educate members
