About The Position

This is a clinical position designed for an NP or PA who wants to practice at the top of their training in a supportive, collaborative environment. You will be integrated into the physician’s practice — not functioning independently without support. The role offers direct mentorship, working closely with a fellowship-trained geriatrician who enjoys teaching and case discussion, providing active collaboration, real-time feedback, and meaningful clinical development. It features a true continuity model, allowing you to see patients in clinic, assisted/independent living, skilled nursing, and after hospital discharge, offering rare exposure to the full arc of geriatric care. The position involves complex medicine, managing polypharmacy, frailty syndromes, dementia care, high-utilizer patients, advanced care planning, and chronic disease optimization. For those interested, a long-term leadership trajectory may include involvement in clinic management, clinical operations strategy, program expansion, and interdisciplinary team development, fostering growth as a clinician and clinical leader in geriatric care. Requirements

  • Board-certified NP or PA
  • Comfortable in multiple care settings

Nice To Haves

  • Interest in geriatrics (experience preferred but not required)
  • Strong clinical rotations in geriatrics (for new graduates)

Responsibilities

  • Co-manage a defined panel of geriatric patients
  • Provide outpatient primary care visits in a geriatric-focused clinic
  • Conduct rounds in Assisted Living and Independent Living communities
  • Participate in Skilled Nursing Facility (SNF) care alongside the physician
  • Perform post-hospital / post-SNF transitional care follow-ups
  • Engage in medication reconciliation, goals-of-care discussions, complex care planning, and comprehensive Annual Wellness Visits
  • Collaborate with facility staff, families, and interdisciplinary teams
