The Nurse Practitioner will provide comprehensive patient care in the home working as the clinical team leader for our home health, palliative care and hospice teams. Requirements
- Master’s degree in Nursing or RN license.
- Advanced Practice Nursing license.
- NP Board Certification from recognized certifying body.
- Unrestricted prescribing and dispensing privileges with DEA licensure required.
- Driving is a requirement for this position. Current state driver's license and proof of insurance is required.
- Strong communication, organizational, and customer service skills.
- Ability to handle a high volume of patients with skill and confidence.
- Knowledge and use of an Electronic Health Record.
- Ability to work with confidential information including HIPAA protected information.
- Ability to be proactive and initiate processes with little to no supervision.
- Fingerprint clearance.
Nice To Haves
- Credentialed with insurance companies preferred.
- 1 to 2 years' NP experience preferred.
- Experience with adult, geriatric populations and population health management preferred.
- Knowledge of Skilled Nursing/Transition Rehabilitation Centers (preferred).
Responsibilities
- Perform comprehensive patient assessments and collaborate with facility staff on preventive services and early changes in condition recognition.
- Manage acute and chronic conditions, including medication adjustments.
- Order and interpret diagnostic tests.
- Develop, implement, and modify treatment plans as needed.
- Participate in quality assurance and improvement activities.
- Provide clinical education and training for patients, families, and facility staff.
- Adhere to state-specific NP scope of practice requirements.
- Demonstrates knowledge of etiologies, risk factors, pathophysiology, and presentation of acute and chronic conditions.
