As an IKC Nurse Practitioner (NP) you will support some of our most complex patients, assisting them in navigating a challenging healthcare system. You’ll focus on delivering holistic, integrated care to improve the quality of life for our kidney patients. The Nurse Practitioner provides comprehensive healthcare, both in-person and via telehealth, across the many co-morbid chronic conditions common to CKD/ ESKD patients within a designated market or geographical region. The NP functions autonomously, yet in collaboration with all members of the healthcare team to provide, coordinate, and facilitate quality, cost-effective care while minimizing fragmentation of the healthcare delivery system for their patients. The NP assumes a clinical leadership role and coordinates activities with the healthcare team and healthcare systems to meet the health needs of their patients.
