As an IKC Nurse Practitioner (NP) you will support some of our most complex patients, assisting them in navigating a challenging healthcare system. You’ll focus on delivering holistic, integrated care to improve the quality of life for our kidney patients. The Nurse Practitioner provides comprehensive healthcare, both in-person and via telehealth, across the many co-morbid chronic conditions common to CKD/ ESKD patients within a designated market or geographical region. The NP functions autonomously, yet in collaboration with all members of the healthcare team to provide, coordinate, and facilitate quality, cost-effective care while minimizing fragmentation of the healthcare delivery system for their patients. The NP assumes a clinical leadership role and coordinates activities with the healthcare team and healthcare systems to meet the health needs of their patients.   • Performs enhanced annual wellness visits or Comprehensive Health Evaluations (CHEs) to identify and evaluate patients’ co-morbid conditions and identify and assist with patients’ needs, including but not limited to medical, social, emotional, and financial concerns through medical record review, consultation with patient, family, and healthcare team members.
  • Manage complex patients, focused on reducing hospitalizations and improved health outcomes
  • Manages the care of CKD/ESKD patients, focusing on delaying disease progression (CKD), medical cost/utilization and preventing/reducing hospitalizations, through proactive post-discharge planning and follow-up with patients most at risk.
  • Will leverage prescriptive authority, medication management, ordering/interpretation of labs/diagnostics, collaboration with other providers, etc
  • Monday- Friday schedule with the ability to accommodate patient and nephrology partner’s availability. Must be flexible.
  • Passion for delivering care to patients with complex, chronic illnesses
  • Minimum of two (2) years' experience as a Nurse Practitioner (NP)
  • Current unrestricted NP license to practice in the state of practice
  • Current Drug Enforcement Administration (DEA) license required or eligible to obtain within 90 days of hire
  • Current Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS) certification
  • Current valid driver’s license and insurance within the state of residence
  • Intermediate computer skills and proficiency in MS Word, Excel & Outlook required
  • Demonstrated proficiency in clinical assessment and management of complex comorbidities, diagnosis, planning, implementation, documentation, and evaluation regarding the needs and care of chronic disease.
  • Strong clinical judgment and confidence in autonomous decision-making while practicing within established protocols and evidence-based guidelines
  • Home Office, High Speed Internet and Reliable Transportation Required.
  • Preferred experience working in Value Based Care or Population Health including documenting and coding from a MA (Medicare Advantage) risk perspective and conducting Health Risk Assessments on high risk/chronically ill patients.
  • 5+ years’ experience as a Family Nurse Practitioner (FNP), Primary Care Nurse Practitioner (NP), or experience in Internal Medicine, Cardiology, Nephrology or Case Management
  • Manage pop. Health (hospitalization)
  • Prior experience with Cerner or similar EHR system
