About The Position

The Nurse Practitioner is a licensed advanced practice clinician responsible for delivering virtual care to patients enrolled in Welby Health’s chronic care management and remote physiological monitoring programs. In this position, you will leverage Welby Health’s platform to conduct structured telehealth visits, review patient data within established clinical protocols, and provide evidence-based interventions that support long-term health outcomes. As a Nurse Practitioner at Welby Health, you will collaborate closely with physicians and cross-functional teams to ensure quality care delivery and regulatory compliance. Furthermore, you will represent Welby Health’s innovative healthcare model to patients and their families, helping them understand how our solutions support long-term health management while empowering them to take an active role in their care.

Requirements

  • Active and valid nurse practitioner (NP) license in the state of Florida
  • Board certification as a family nurse practitioner (FNP), adult-gerontology nurse practitioner (AGNP), or equivalent
  • Eligibility for Medicare provider enrollment and participation
  • Bachelor’s degree in nursing or a closely related healthcare field
  • Exceptional written and verbal communication skills
  • Ability to work independently in a remote, technology-enabled environment
  • Strong organizational skills with attention to detail and follow-through

Nice To Haves

  • Experience in case management, chronic care management, and/or telehealth
  • Multi-state licensure for expanded practice flexibility
  • Drug Enforcement Administration (DEA) licensure
  • Bilingual or multilingual proficiency
  • Familiarity with remote physiological monitoring, electronic medical record (EMR) or electronic health record (EHR) systems, and patient engagement tools

Responsibilities

  • Monitor and interpret patient vitals, assessments, and alerts within Welby Health’s platform
  • Conduct virtual patient assessments, medication reviews, and care plan evaluations
  • Provide timely, evidence-based guidance and escalate clinically significant findings to supervising physicians when appropriate
  • Document patient interactions accurately and completely in accordance with internal protocols and regulatory standards
  • Coordinate with care teams and physicians to ensure seamless care delivery and continuity
  • Support quality improvement initiatives through consistent, high-quality clinical documentation and adherence to established workflows
