We are looking for a passionate Nurse Practitioner NP to join our leading team of hospice care professionals and make a difference, by listening, caring, and serving. POSITION SUMMARY: The Nurse Practitioner works in collaboration with a physician and other palliative care team members to deliver palliative care and related health care services to chronic and serious ill patients and families.

Requirements

Graduate of an accredited School of Nursing and accredited Nurse Practitioner Program.
Current state RN and APN license and CPR certification.
Minimum 2 years’ experience as an APN clinical care setting preferred; hospice and home health preferred.
Excellent observation, verbal and written communication skill, problem solving skills, nursing skills per competency checklist.
Reliable transportation, valid driver’s license, and current auto liability insurance coverage.
