About The Position

US Heart and Vascular is seeking a dedicated Nurse Practitioner to join our Value Base Care Integrated Care Clinic, a critical role in our multidisciplinary team focused on optimizing outpatient care for heart failure patients. Position Highlights: Full-time position in a collaborative clinic environment Monday – Friday No weekend/no call Excellent benefits package Responsibilities & Duties: Under the supervision of a physician: Comprehensive management of patient panel and the patient conditions, including assessment, diagnosis, treatment, and consultation, on a longitudinal basis Comprehensive documentation of patient chronic conditions Leverage remote patient monitoring technology along with appropriate diagnostic tests to monitor patients’ disease states and care Guideline directed medical therapy including prescribing, titration, and management for various cardiovascular disease states Assess all patient and caregiver needs to establish goals of care and collaborates with interdisciplinary care team to manage patients care, including addressing social determinants of health, advanced care planning, transitions of care, and patient and caregiver education Assesses patients' heart failure status and implement interventions to reduce acute care needs inclusive of addressing fluid volume overload in outpatient clinic setting Assess patients’ cardiology disease state status and refer for health optimizing procedures such as ICD placement, ablation, and revascularization procedures as appropriate About Tucson, AZ: Tucson offers stunning desert landscapes, mountain views, and over 300 days of sunshine. Requirements

  • Master’s degree in nursing and completion of an accredited nurse practitioner program
  • Certified Nurse Practitioner (NP) with an active AZ state license or willing to apply
  • BLS and ACLS certifications
  • Active AL license or willing to apply
  • NP Board Certification

Nice To Haves

  • ARNP Experience in a Heart Failure or Complex Cardiology Program or two (2) years of experience working in a cardiovascular setting is preferred but not required
  • Open to new ACNP grads with cardiology experience as a nurse
