We are part of American Health Partners, which includes rapidly growing Medicare Advantage plans, psychiatric care, and pharmacy services. We are seeking an advanced practice provider, either a nurse practitioner (NP) or physician assistant (PA), with experience in primary care or internal medicine, ideally with a focus on geriatrics. This role offers the opportunity to significantly improve outcomes for long-term care residents by collaborating with a team of providers and case managers. Our providers primarily work within nursing homes or assisted living communities, caring for residents enrolled in special Medicare Advantage plans. You will be responsible for delivering personalized, coordinated health care to plan members, aiming to enhance their quality of life and prevent unnecessary hospital visits. As an advanced practice provider, you will exercise independent judgment to treat patients with acute and chronic conditions, consulting with a supervising physician when appropriate. You will also work closely with our facility partners and health plan leadership to implement our national model of care, ensuring compliance with all federal and state regulations.
