About The Position

The Nurse Practitioner is an integral member of the provider team, provides direct medical services to patients under direction and responsibility of a physician.

Requirements

  • Graduate of an accredited training program for APRN.
  • Certified by the state of Oklahoma to practice as a Nurse Practitioner.
  • Eligible for medical malpractice insurance.
  • Holds current continuing CME requirements and other credentialing and privileging criteria required for this position.
  • Possesses DEA and Oklahoma State Bureau of Narcotics and Dangerous Drug Control Certificate.
  • Holds Basic Life Support (BLS) Certification.
  • Basic computer skills needed.
  • Physical and mental capability to perform the responsibilities of the position.

Responsibilities

  • Provides direct patient care, performs comprehensive physical examination, obtains a thorough and appropriate health history, compiles patient medical data and arranges testing, therapy, and follow-up.
  • Administers or orders diagnostic tests, such as x-ray, electrocardiogram, and laboratory tests, and interprets test results for deviations from normal.
  • Performs therapeutic procedures such as injections, immunizations, suturing and wound care, and managing infection.
  • Develops and implements patient management plans, records progress notes, and assists in provision of continuity of care.
  • Instructs and counsels patients regarding compliance with prescribed therapeutic regimes, normal growth and development, family planning, emotional problems of daily living, and health maintenance.
  • Documents each patient encounter in the patient record, clearly and concisely, on the day the patient encounter takes place.
  • Serves as a participant on rotational call schedule with other clinicians.
  • Participates in provider team meetings to comply with PCMH team standards.
  • Participates in any other designated activities and projects for PCMH as assigned by the Chief Medical Officer.
  • Initiates patient referrals as necessary, and communicates with the referral clinician or agency.
  • Supports activities related to the Health Disparity Collaborative. Duties include but are not limited to supporting the incorporation of staff time for data entry, team meetings, production and submission of monthly reports, maintenance of database.
  • Maintains and improves clinical skills by attending health center provider meetings.
  • Consults supervising or alternate physician when appropriate.
  • Participates in community agencies and/or resources in providing continuity of care.
  • Maintains up to date required state licensures and certifications.
  • Follows the mission and vision statement.
  • All other duties as assigned.
