Ready to Make a Meaningful Impact in Home-Based Care? We are seeking compassionate and driven Nurse Practitioners to join our dynamic Clover Home Care teams in Albany, Georgia . This is a unique opportunity to provide comprehensive primary and supportive care directly in the homes of some of our most vulnerable members, helping to improve health outcomes and quality of life. As a Clover Home Care Nurse Practitioner, you’ll join our Clover Care Visit (CCV) Team. The CCV team focuses on early intervention and transitional care, helping to set the foundation for long-term health and reduce avoidable complications. You’ll work closely with our clinical insight platform, Clover Assistant, to support decision-making and improve outcomes.

Requirements

  • You are located in or near Albany, Georgia and able to travel locally for home visits.
  • You have an active and unencumbered GA Registered Nurse license.
  • You are a Certified Advanced Practice Nurse Practitioner (APN).
  • You have a master’s degree or post-master’s certificate in advanced clinical practice.
  • You have a minimum of 1 year of NP experience in a clinical setting.
  • You have active DEA and CDS registration.
  • You have current Basic Life Support (BLS) certification.

Nice To Haves

  • You have experience in home care or house call settings (preferred, but not required).

Responsibilities

  • Conduct comprehensive in-home health assessments for enrolled Medicare Advantage members.
  • Perform post-discharge follow-up visits to reduce hospital readmissions.
  • Collaborate with interdisciplinary teams using data-driven clinical tools.
