About The Position

Horizon Health is seeking a Nurse Practitioner to join the team at Valle Vista for our opioid clinic called New Vista in greenwood, IN. The OTP Nurse Practitioner (NP) will work under the supervision of the Opioid Treatment Program (OTP) Medical Director to provide comprehensive care to patients in an OTP. The NP will perform patient assessments, intake patients into the program, and carry out other standard functions of a Nurse Practitioner within the opioid treatment setting. The OTP Nurse Practitioner will conduct thorough patient assessments, including medical history, physical examinations, and diagnostic tests. The OTP Nurse Practitioner will also perform patient intake procedures, including initial evaluations and development of individualized treatment plans and administer medication-assisted treatment (MAT) for opioid use disorder, including prescribing and managing medications such as methadone, buprenorphine, and naltrexone.

Requirements

  • Master's Degree and current NP licensure in Indiana required
  • Experience and/or speciality training in addiction medicine or addiction psychiatry required
  • A working knowledge of accreditation and licensing standards associated with the CARF, DEA, DBHDS, DMAS/ARTS, SAMHSA and Board of Pharmacy preferred.
  • A valid driver's license and proof of insurance required

Responsibilities

  • Monitors patient progress and adjust treatment plans as necessary.
  • Educates patients and their families about opioid use disorder, treatment options, and recovery strategies.
  • Collaborates with the multidisciplinary team, including physicians, counselors, and social workers, to ensure coordinated and comprehensive care.
  • Maintains accurate/up-to-date patient records in compliance w/ federal/state regulations.
  • Participates in quality improvement initiatives and continuing education to stay current with best practices in opioid use disorder treatment.
  • Addresses barriers to treatment, such as stigma and financial constraints, by providing supportive, nonjudgmental care.
  • Provides consultation to after-hours medical or mental health emergencies.
  • Provides supervision of Dosing Nurses as necessary.
  • Monitors and evaluates patients in recovery & provide interventions that will eliminate or reduce Substance Abuse.
  • Consistently and objectively intervenes with staff to correct employee performance that does not meet expectations using coaching or disciplinary counseling techniques.
  • Conducts and maintains minutes of monthly departmental meetings.
  • Develops and maintains staff schedule that is consistent with facility and departmental needs.
  • Develops and maintains clinical staff who are competent to provide departmental services.
  • Coordinates discipline-specific performance improvement activities as indicated.
  • Participates in hospital-wide performance improvement activities as indicated.
  • Completes and/or monitors time and attendance for clinical staff in compliance with Foundations Health policy and procedure.
