This is a full-time, outpatient-focused role providing medical care to patients in skilled nursing facilities and home-based settings . The Nurse Practitioner will manage acute and chronic conditions for elderly and medically complex patients, with a strong focus on continuity of care, preventive services, and reducing unnecessary hospitalizations. Requirements
- Graduate of an accredited Nurse Practitioner program (MSN or DNP)
- Active or eligible New York State Nurse Practitioner license
- National certification required (FNP, AGNP, or GNP preferred)

Responsibilities
- Provide comprehensive medical care to patients in nursing homes and home-based settings
- Perform history and physical examinations, routine follow-ups, and urgent evaluations
- Diagnose and manage acute and chronic medical conditions
- Order, interpret, and evaluate diagnostic tests and labs
- Develop and manage individualized treatment and care plans
- Coordinate care with primary care physicians, specialists, nursing staff, and caregivers
- Educate patients and families on disease management, medications, and preventive care
- Maintain accurate and timely documentation in the electronic health record (EHR)
- Participate in care coordination, quality improvement initiatives, and population health efforts
