Nurse Practitioner – In-Home & Telehealth Care
About The Position
ACE Physicians is expanding into Tennessee, and we are looking for compassionate and motivated Nurse Practitioners to help bring high-quality, patient-centered care directly to the communities we serve. ACE Physicians is the sister company of Varco Healthcare, an established healthcare organization focused on delivering innovative and accessible care models. This hybrid role provides care through in-home visits and telehealth, allowing providers to build meaningful relationships with patients while delivering personalized care in the environments where patients feel most comfortable. Requirements
- Active, unrestricted Nurse Practitioner license (required).
- Excellent communication skills and a collaborative, team-oriented mindset.
Nice To Haves
- Experience in geriatrics, internal medicine, or primary care preferred.
- Previous home health or home-based care experience strongly preferred.
- Experience caring for patients with disabilities is highly desirable.
- Comfortable delivering care in both in-home and telehealth settings.
Responsibilities
- Perform comprehensive health assessments and physical examinations during in-home visits and telehealth appointments.
- Evaluate, diagnose, and treat acute and chronic conditions with an emphasis on preventive care.
- Develop and implement individualized treatment plans tailored to each patient’s needs.
- Educate patients and families on health management, wellness, and disease prevention.
- Collaborate with physicians, specialists, and interdisciplinary care teams to ensure coordinated care.
- Perform minor clinical procedures such as wound care as appropriate during home visits.
- Maintain accurate and timely documentation in electronic medical records (eClinicalWorks or similar platforms).
- Advocate for patients by providing respectful, inclusive, and equitable care, including individuals with disabilities.
