What You’ll Do
Provide care in nursing and long-term care facilities
Manage chronic conditions, perform full assessments
Close care gaps (HEDIS and STAR measures)
Lead care planning with facility teams
Support and educate patients’ families

Requirements

• Active, unrestricted APN license in Michigan
• Board certified and Medicare credentialed
• 6+ months in skilled nursing or long-term care
• Solid with digital tools, team-based care
• Able to work onsite in Midland, MI
