About The Position

The Nurse Practitioner is responsible for recording physical findings, formulating patient care plans and prognoses based on the patient's condition. This role involves discussing cases with physicians and other health professionals to develop comprehensive patient care plans, prescribing or recommending drugs or other treatments, and monitoring and maintaining patient progress while reporting to the supervising physician. The Nurse Practitioner is expected to work collaboratively with hospice employees and volunteers as part of an Interdisciplinary Team (IDT).

Requirements

• minimum of (2) years experience in hospice, home health or community health in the past five (5) years.
• completion of accredited Nurse Practitioner program.
• State licensure is also required.
• ability to assist in lifting and transferring patients as required.
• ability to lift objects in excess of 20 pounds.
• Specific vision abilities required by this job include: close vision, distance vision and ability to adjust focus.
• ability to stand or walk in excess of one hour per day, talk, hear, sit, stoop, kneel and crouch as needed.

Responsibilities
