He/She provide s hospice car e t h rough th e evaluation an d asses s ment o f the healt h hist o r y , health pr o motio n , diagnosing medical con d itions, manageme n t of h ealth probl e ms b y dire c ting and developing the plan of c a re, prescribing medications o r tre a tments, co o rdinating an d collaborating wit h patients, families and o ther healt h care p ract i tioners. Under th e supervision o f a ph y sician, th e nurs e practition e r visit s patie n ts, c o mplete s physical a s sessme n ts, perform s fa c e t o fac e assessments and evaluate s hospice e l igibility as assigne d in c o nsultation wit h th e hosp i ce interdisciplinary team. Th e nurs e p ractiti o ner builds fr o m the reso u rces o f th e communit y t o plan and dir e ct services to me e t the ne e d of individuals and f a milies within their hom e s a n d communitie s .

Requirements

  • Gradu a tion fr o m a n ac c redited School o f Nursing and ac c redite d Nurs e Practitioner Program.
  • Current nursing licensure in St a te an d CPR certification.
  • Mast e r' s de g ree wit h a minimum o f on e ( 1 ) ye a r N urse P ractition e r exp e rience and a minimum of one (1) ye a r hospice or palliative car e experienc e .
  • Accreditatio n from an a p proved c e rt i fying bod y f o r advance d pr a ctice nur s ing as requ i red by St a te.
  • Excellent observ a tion, v e rbal and w ritte n c o mmu n ication skill, probl e m sol v ing skills, math e matical skills; nurs i ng skills per comp e tenc y checklist.
  • Prolonged or considerab l e walking or standing. A ble to lift, position an d / o r tran s fer patients. Able to lift supp l ies and equipme n t. Con s iderable reaching, stoop i ng, bending, kneeling and/or crouchi n g. Visual acuit y an d hearing to p e rf o rm requ i red n ursing skills.
  • Must be a licensed d river with a n aut o mobile th a t i s insure d in acc o rdance with st a te/ o r organization requireme n ts and i s in good w o rking order.

Responsibilities

  • Provides medical car e , p alliates and manag e s terminal illness in complia n ce with the s t a te Nurse Pra c tice A c t un d er th e supervision o f a col l aborating ph y sician.
  • Completes hi s tory and p hysical examination s by evaluating th e patient' s medical condition and health hi s tory, o rder i ng and int e rpr e ting results from labor a tory and di a gnostic tests or procedure s , diagnosing health conditions a nd d o cuments the findings in the patient' s medical rec o rd.
  • Manages hea l th cond i tions and terminal illnesses by developing a n individualized plan of care, pre s cribing medications and / or treatment s , obtaining consult a tions or making ref e rrals an d co o rdinating hospice c a re s e rvice s .
  • Completes the h o spice face t o f a ce visit requir e ments p rior to th e thir d an d subsequent hospice benefit period s to asses s hospice eligibility base d o n th e hospice conditions of participation an d th e loc a l coverag e det e rminatio n s. P rovides clinical information to th e hospice medical dire c t o r for inter p ret a tion for eligibility for hospice recertif i cation.
  • Make s rec o mme n datio n s t o th e hospice medical director reg a rding p a tient eligibility f o r certific a tion and rec e rtifi c ation f o r hospic e care.
  • Reviews clinical information, coordin a tes an d ov e rsees p a tient car e in co n sultation wit h th e hospice interdisciplinary team and o ther healthc a re pra c titioners.
  • Assures that p a tient rec e ives appro p riate meas u res t o co n trol s y m p toms, through collaboration with int e rd i sciplinary te a m memb e r s .
  • Communicat e s an d collaborate s with the i n terdis c iplinary group to create review and revise the p a tient' s plan of c a re.
  • Consults with the p a tient's attending physician, t h e hospice medical dire c tor an d oth e r healthcare p ra c titioners regarding th e patie n t' s eligibility for h o spice c a re, ongoing care needs and medical ma n agement relate d to the p atient' s t e rminal condition.
  • Communicat e s wit h oth e r c o mmun i ty hea l th p ractitioners to co o rdinat e th e plan of car e .
  • Attend s and p a rticipate s in hospice interdisciplinary g roup meeting s .
  • Completes, maintains a n d subm i ts a c cur a te an d relevant clinical note s , p h ysical examination n o tes, ass e ssme n t visit note s , med i cal orders, collaboration notes an d oth e r document a tion in the m e dical recor d .
  • Educates/co u nsels p a tients, f a milies, an d /or car e givers a s t o preve n t a tive care, medical problems, ps y chological problems, an d spiritual p roblems i n conj u nction w ith th e interdisciplinary te a m t o meet th e t o tal need s of p atients.
  • Provides and maintain s a saf e enviro n ment f o r t h e patient.
  • Assists the p a tient and family/caregiv e r an d oth e r team m e mb e rs in p rov i ding continuity o f care.
  • Work s in coop e ration with th e f a mily/caregiv e r a n d hospice int e rdisciplinary grou p t o identify the goal s of car e and me e t the car e need s of th e patient an d f a mily/caregiver.
  • Serves a s a n u rsing res o urce for consult a tion an d education s t o m e mb e rs of the interdisciplinary te a m an d oth e r healt h car e pra c titioners in the c o mmunity.
  • Establishes, builds an d n urt u res relationships wit h st a ff and communit y r e ferral so u rce s t o facilitate p rogr a m g rowt h .
  • Maintains knowledge o f and compliance wit h c u rrent Medicar e /Medicaid, s tat e /fed e ral rule s and regulation s f o r hosp i ce services
  • Ensures c o mpliance wit h th e Medic a re condition s of p a rticipation an d oth e r st a te regulations gov e rn the p rovision of healt h car e .
  • Complies with all Health Insuranc e Portability an d Accountability A c t (HIP A A) requir e me n ts in accordanc e with federal, st a te and o rganizati o nal policies.
