About The Position

Humana's Primary Care Organization, operating as CenterWell & Conviva, is one of the largest and fastest-growing value-based care, senior-focused primary care providers in the country, with over 400 centers across 15 states. This specific role is a float position within the Las Vegas, NV market, supporting IPA (Independent Practice Association) needs. The IPA Nurse Practitioner will be clinical, reporting to the Associate Medical Director and collaborating with the Area Director of Operations and Provider Engagement Professional Team. The primary focus will be to train, support, and perform annual wellness visits in assigned IPA outpatient clinics, emphasizing Medicare documentation guidelines and HEDIS/Quality measures. The role also involves developing and enhancing clinic workflows to align with Value-based Care delivery models, ultimately aiming to improve patient outcomes. CenterWell Senior Primary Care provides proactive, preventive care to seniors, including wellness visits, physical exams, chronic condition management, screenings, and minor injury treatment. Their unique care model focuses on personalized experiences, with integrated care teams spending more time with patients to address physical, emotional, and social well-being. Requirements

  • Three years of experience
  • Successful completion of a Nurse Practitioner/Physician Assistant program with Board Certification
  • Active, unrestricted NP/PA license in state of practice
  • Active and unrestricted DEA license
  • Undergo TB screening (if selected for the role)

Nice To Haves

  • Knowledge of Medicare guidelines and coverage
  • Knowledge of HEDIS quality indicators

Responsibilities

  • Complete Annual Wellness Visits
  • Improve Quality and Care management
  • Collaborate with population health and the quality department to improve workflows
  • Close quality gaps, improve identification and capture of HCC codes, and improve medication adherence
  • Provide elbow support and training to IPA Physicians and Advanced Practice Providers on Medicare documentation guidelines, HEDIS/Quality measures, and clinic workflows that align with Value Based Care delivery models
