About The Position

Humana's Primary Care Organization, operating under the CenterWell and Conviva brands, is a large and rapidly expanding senior-focused, value-based care provider across 15 states with over 340 centers. The organization aims to transform healthcare by prioritizing seniors. They are seeking a Primary Care Nurse Practitioner for a PRN (as needed) associate role. This position is suited for a compassionate, experienced clinician who thrives in a collaborative, patient-centered environment and is dedicated to improving outcomes for adult and geriatric populations. The role emphasizes team-based care, focusing on whole-person care (physical, emotional, social), allowing more time with patients due to lower daily patient volume, and fostering a supportive, inclusive culture that values teamwork, innovation, and learning. CenterWell Senior Primary Care provides proactive, preventive care to seniors, including wellness visits, physical exams, chronic condition management, screenings, and minor injury treatment. Their unique care model focuses on personalized experiences, taking time to listen, learn, and address factors impacting patient well-being. Integrated care teams, including physicians, nurses, and behavioral health specialists, spend up to 50 percent more time with patients to provide compassionate, personalized care for better health outcomes, addressing physical, emotional, and social wellness.

Requirements

  • Master's degree in nursing with board certification.
  • Active, unrestricted NP license in the state of practice.
  • Minimum two years' Nurse Practitioner experience.
  • Commitment to improving patient experience and outcomes.
  • EMR experience and adapts well to new technologies.

Nice To Haves

  • Experience in Value-Based care, Primary Care or Medicare Advantage settings working with Geriatric patients.
  • Proficiency with electronic health records Athena Health and digital documentation.
  • Active DEA license and Medicare/Medicaid provider numbers or ability to obtain DEA license or Medicare and Medicaid numbers.
  • Familiarity with HEDIS quality indicators and Medicare guidelines.

Responsibilities

  • Deliver comprehensive outpatient care to adult and senior patients.
  • Maintain accurate medical records and documentation.
  • Diagnose and manage moderately to complex medical conditions.
  • Coordinate referrals and collaborate with specialists.
  • Participate in clinical quality improvement programs.
  • Work with interdisciplinary teams to ensure holistic care.
