About The Position

Under the direction of the medical director, employee provides general medical care and treatment to Avow’s patients in their homes, hospitals, nursing care facilities, assisted living facilities, outpatient and inpatient settings.

Requirements

  • Master's degree (M.S.) or post-Master’s Degree.
  • APRN license, State of Florida.
  • Proof of national nurse practitioner certification.
  • Current DEA Registration
  • Upon hiring either have a Certification in Hospice and Palliative Care (ACHPN) or pursuing certification and have completed by 2 years of employment.
  • Ability to read, analyze, and interpret common scientific and technical journals, financial reports, and legal documents.
  • Ability to respond to common inquiries or complaints from customers, regulatory agencies, or members of the business community.
  • Ability to write speeches and articles for publication that conform to prescribed style and format.
  • Ability to effectively present information to top management, public groups, and/or boards of directors.
  • Math skills including addition, subtraction, multiplication, division, and percentages/fractions.
  • Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations.
  • Ability to apply principles of logical or scientific thinking to a wide range of intellectual and practical problems.
  • Ability to use hands to finger, handle, or feel and talk or hear.
  • Ability to stand, walk, and reach with hands and arms.
  • Ability to lift and/or move up to 50 pounds and occasionally lift and/or move up to 100 pounds.
  • Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus.

Responsibilities

  • Works in collaboration with the physician and other team members to provide patient/family centered care to meet the needs of patients and their families/caregivers including assessment, psychosocial support and counseling, advanced care planning, family conferencing, education and pain and symptom management.
  • Utilizes the electronic medical record system and documents appropriately at bedside and submits all documentation by the end of the business day.
  • Performs physical examinations within scope of practice.
  • Orders, interprets, and evaluates diagnostic tests to identify and assess patient's clinical problems and health care needs as appropriate.
  • Provides skilled, effective interventions that meet the patient’s needs.
  • Respects the patient’s beliefs, values and goals for care.
  • Collaborates with the patient’s attending physician, the interdisciplinary care team and community health care providers to achieve outcomes that meet the patient’s needs.
  • Offers a caring presence to the patient and family and provides ongoing emotional support.
  • Supports the patient’s search for a renewed sense of meaning, purpose and hope.
  • Serves as an advocate to help patients and families receive needed services.
  • Participates in teaching activities to improve the standard of patient care.
  • Attends interdisciplinary team (IDT) meetings and participates in the IDT process as requested by team supervisor.
  • Assists in the admissions process in determination of patient medication and medical needs.
  • Prescribes or recommends drugs or other forms of treatment that are appropriate with patient’s plan of care.
  • Refers patients to physician for consultation as appropriate.
  • Takes team calls for the organization as requested and scheduled.
  • Provides on-call support on an alternating basis, to the interdisciplinary team, patients, families and patient’s primary physicians in order to provide 24 hour, 7 day per week medical care for our patients.
  • Acts as clinical resource to staff.
  • Performs patient visits in all areas requested within scope of practice.
  • Other duties as assigned.
