About The Position

Per Diem Rates: The telehealth visits for community = $150 (which is an average compensation). In person visits are compensated = $150- $210 (with $180 being an average FFS) Orientation hourly rate: $90/Hour This field position will cover Brooklyn and Queens regions. Requirements

• Masters Degree in Nursing, graduate of accredited Nurse Practitioner program required
• One to two years clinical experience in role of nurse practitioner required
• NYS NP License, Registration, DEA certificates
• Driver license and car required

Nice To Haves

• Minimum one year in geriatric specialty
• Previous clinical RN experience in acute, subacute and/or long term care setting preferred
• Prior experience in manage care a plus

Responsibilities

• In collaboration with physician, the NP will provide comprehensive assessment, including multifactorial complex physical examination and psychosocial needs, self - management education and tools to better manage patient’s multiple conditions.
• As a part of Interdisciplinary Co-Management Program, the NP will be providing and coordinating primary care services to an assigned the at risk panel of Elderplan members who reside in the community.
• The NP will insure development of care planning and access to preventive care services in accordance with member and Plan health outcome goals and objectives.
• The NP will act as clinical resource to the member, family and interdisciplinary team.
• Provide and/or coordinate primary clinical care services for assigned high risk members and facilitates supportive care during transitions from acute or sub-acute care setting
• Acts as a team player, member’s advocate, and a clinical lead while working closely with Interdisciplinary Team to coordinate member’s care and optimize health outcomes
• Acts as resource and contact for member/designated representative regarding all aspects of member individualized care plan and disease process
