About The Position

Our client is dedicated to transforming the landscape of mental health care for individuals coping with Serious Mental Illness (SMI). They uphold the belief that these individuals should receive greater emphasis in care and treatment. The Nurse Practitioner who joins their team will head the medical aspects of the team-based care model we have adopted for individuals with serious mental illnesses. The role involves documenting and addressing the medical necessities of the individual and performing comprehensive clinical assessments to identify and address any gaps in care. This role is also tasked with coordinating care amongst the individual's medical and behavioral health providers, making appropriate referrals, and providing clinical support to Guides (peers) and Community Resources Guides (social workers).

Requirements

  • Hold a Master's degree or above.
  • Bring a minimum of 3 years of experience within Primary Care or Value-Based Care.
  • Hold a valid Nurse Practitioner (NP) certification/license in Virginia (VA).
  • 5+ years of clinical care as a Nurse Practitioner, ideally in a primary care setting.
  • Experience working with underserved populations, particularly those with behavioral health and/or SUD conditions.

Nice To Haves

  • Experience with HCC Coding would be advantageous.

Responsibilities

  • Conduct a complete-person clinical assessment.
  • Mentor the care team on various clinical issues and treatments.
  • Lead medical segment of holistic care, identifying actionable care gaps and coordinating care.
  • Conduct visits at field locations or individuals' residences as required.
