About The Position

Nurse Practitioner, Full-Time, Day Shift Community Care is hiring a Nurse Practitioner for our Pace Program. Requirements

  • Education: Masters degree in Nursing required.
  • Licensure: Wisconsin Registered Nurse/RN license required. Family, Geriatric, or Adult NP certification is required. DEA registration required. Advance Nurse Prescriber required. NPI and CPR & AED required
  • Experience: minimum of 1 year recent clinical nursing experience with an at risk or elderly population.
  • Monday - Friday, 40 hours per week
  • 8:00AM - 4:30PM
  • On-Call responsibilities: 6-8 weeks per year required

Responsibilities

  • Documents and updates health history and findings, physical exam, functional status evaluations, diagnostic test and lab evaluations, mental health status and function evaluations, preventative care, and health reminders.
  • Within their official scope and practice, delegates appropriate tasks to interdisciplinary team staff, and provides leadership and guidance. Acts as a resource to staff to ensure appropriate care and follow through and provides care planning assistance.
  • Educates members and families regarding health maintenance and acute and chronic illnesses.
  • Travel, Regular local travel, requiring use of personal vehicle, valid driver's license, and proof of required insurance coverage.
