About the Role: Nurse Practitioner $140/visit Travel: Southwest WA Traveler. Covers Clark, Cowlitz, Lewis (Stevenson, Kelso and Chehalis) In-home, local travel You choose your hours (min. 24/week). Just assessments, no meds, no Rx, no clinic.
Requirements
- Must have license/certification: APRN, Certified Nurse Practitioner
- Must be registered with Medicare and/or Medicaid
- Valid driver’s license + reliable car
- Able to work independently in the field
Responsibilities
- Complete 6–9 in-home health assessments/day
- Review of medical history and risk factors
- Collect vitals, labs, and screening data
- Document findings in EMR
- Educate members on next steps
- Coordinate follow-up if red flags show
- Travel across Southwest Washington (Clark, Cowlitz, and Lewis Counties)
