About The Position

We are seeking a Nurse Practitioner to join our Healthcare Partnership Program (HPP). In this role, you will be placed onsite at a partner organization to provide high-quality primary and preventive care to employees, spouses, and dependents. This position is ideal for a clinician who enjoys working autonomously while remaining supported by a collaborative healthcare team. You’ll focus on proactive health management, chronic condition support, and addressing urgent health needs — all within a workplace setting designed to remove barriers to care.

Requirements

• Master’s degree in Nursing from an accredited program
• Active, unrestricted State of Michigan Registered Nurse License
• Active, unrestricted Michigan Nurse Practitioner Specialty Certification
• Active FNP national board certification through: American Academy of Nurse Practitioners (AANP), or American Nurses Credentialing Center (ANCC)
• Minimum 2 years of nursing experience in applicable settings (preferred)
• BLS Certification from AHA or Red Cross (or obtained within 90 days of hire)

Nice To Haves

• DOT Examiner Certification (training available if needed)
• Additional certifications or experience in areas such as:

• Health Coaching
• Case Management
• Women’s or Men’s Health
• Sports Medicine
• Hospital-based specialties (Med-Surg, ICU, ED)

Responsibilities

• Provide advanced clinical care including acute/episodic care, preventive services, and chronic disease management
• Deliver care to HPP and school-based patients as applicable
• Collaborate with HPP nurses on claims analysis and care planning
• Establish and follow clinical pathways and treatment protocols
• Conduct telehealth visits for HPP patients as needed
• Coordinate care with patients’ external medical providers and networks
• Serve as a clinical consultant while maintaining high standards of care
• Maintain all required licensure and certifications
