Adobe Population Health is hiring a part-time Nurse Practitioner (NP) to conduct in-home wellness assessments for both pediatric and adult members. This role focuses on improving health literacy and overall quality of life through comprehensive annual visits. The NP will complete in-home annual wellness assessments (which may last up to one hour) and will evaluate medical history, current medications, health status, and social determinants of health. This part-time position requires a minimum commitment of 20 hours per week. Availability can include the following: Weekdays: Monday through Friday, 8:00 a.m. – 8:00 p.m. Weekends: Saturday and Sunday, 8:00 a.m. – 5:00 p.m. This position is pay-per-visit and will vary from $75-$125, depending on the type of visit. Travel reimbursement is also included at the IRS-approved rate of .70/mile.

Requirements

  • One (1) year of experience as an NP is preferred.
  • Computer skills required: (Microsoft Office Suite Software).
  • Adapts to changes in policies, procedures, new techniques, & additional responsibilities.
  • Knowledge of HIPAA regulations.
  • Master's Degree in Nursing is required.
  • Current and unrestricted licensed Family Nurse Practitioner.
  • Current and unrestricted Board Certification.
  • Must maintain active certification in designated nurse practitioner specialty as required by certifying body and state regulations.
  • Current CPR certification.
  • Current BLS certification.
  • Current and unrestricted California driver's license.
  • Annual TB testing.

Nice To Haves

  • Experience in the care of elderly population is preferred.
  • Experience in the care of pediatric populations preferred.
  • Experience in home health, physician’s office, or public health is preferred.
  • Bilingual (English/Spanish) is preferred.
  • Clean driving record.

Responsibilities

  • Travel up to 90% throughout designated regions, which may be updated based on business needs.
  • Conduct in-home assessments on identified patients, following established guidelines.
  • Visit types may include:
  • In-home Medicare and Medicaid Assessments.
  • In-home Well-Child Visits (WCV), for children ranging from 6 months to 21 years of age.
  • In-home Child and Adolescent Well-Care Visits (WCV).
  • In-home Annual adult Wellness Visits (AWV).
  • In-home Health Risk Assessments (IHRA).
  • Telephonic Adult Annual Preventative (AAP) Assessments.
  • Telehealth assessments, as needed.
  • Understand the importance of preventative health evaluations focused on monitoring physical, developmental, behavioral, emotional health, and the assessment of social determinants of health (SDOH).
  • Assess psychosocial and clinical needs of patients and families.
  • Coordinate and communicate care plans with primary care and community providers.
  • Support appropriate utilization of healthcare resources.
  • Ensure timely and accurate documentation across the continuum of care.
  • Formulate a list of current and past medical conditions using clinical knowledge, judgment, and current findings.
  • Identify urgent and emergent situations for proper intervention.
  • Educate members on topics related to health conditions and the continuation of care.
  • Identify diagnoses to be used in care management and active medical management.
  • Provide care coordination to alleviate identified needs related to behavioral, physical, and social issues.
  • Focus on addressing issues that will decrease medical costs and increase the quality of life.
  • Develop positive relationships with clients, caregivers, providers, and other team members.
  • Managing physical health problems through the provision of education.
  • Communicate gaps in care to the member’s primary care physician if needed.
  • Home safety evaluations.
  • Identify internal and external referral needs when available.
  • Compliant with all HIPAA regulations and maintain the security of protected health information (PHI).
