About The Position

This is a hybrid clinical position designed for an NP or PA who wants to practice at the top of their training in a supportive, collaborative environment. You will: Co-manage a defined panel of geriatric patients Provide outpatient primary care visits in a geriatric-focused clinic Conduct rounds in Assisted Living and Independent Living communities Participate in Skilled Nursing Facility (SNF) care alongside the physician Perform post-hospital / post-SNF transitional care follow-ups Engage in medication reconciliation, goals-of-care discussions, complex care planning, and comprehensive Annual Wellness Visits Collaborate with facility staff, families, and interdisciplinary teams You will be integrated into the physician’s practice — not functioning independently without support. Requirements

  • Board-certified NP or PA
  • Interest in geriatrics (experience preferred but not required)
  • Comfortable in multiple care settings
  • Strong communicator with patients and families
  • Detail-oriented and thoughtful clinician
  • Values mentorship and team-based practice
