About The Position

One Health is a forward-thinking healthcare organization delivering high-quality, patient-centered care across the continuum of senior and post-acute services. We are seeking a compassionate, knowledgeable, and proactive Nurse Practitioner to provide comprehensive clinical care within our partnered Skilled Nursing Facilities (SNFs). This role is essential to ensuring continuity of care, reducing avoidable hospitalizations, and supporting residents through evidence-based management of chronic and acute conditions. As part of our post-acute care team, you will work closely with facility staff, attending physicians, and interdisciplinary care teams to enhance resident outcomes and strengthen overall quality of care.

Requirements

  • Active Nurse Practitioner License in Illinois (required).
  • Master’s Degree in Nursing (required).
  • 2+ years of experience in primary care, geriatrics, internal medicine, or post-acute/SNF settings preferred.
  • Strong clinical assessment, diagnostic reasoning, and care-planning skills.
  • Familiarity with regulatory standards relevant to SNFs (CMS guidelines, MDS processes, readmission protocols, etc.).
  • Excellent communication, collaboration, and interpersonal skills with a resident-centered mindset.
  • Ability to work autonomously while functioning within an interdisciplinary care model.

Nice To Haves

  • Experience with post-acute, long-term care, or geriatric populations strongly preferred.

Responsibilities

  • Provide comprehensive on-site primary and post-acute care to residents in skilled nursing facilities, including chronic disease management, acute issue evaluation, and routine follow-up visits.
  • Conduct physical examinations, order and interpret diagnostic testing, and prescribe medications in accordance with state and federal regulations.
  • Develop, implement, and monitor individualized care plans aligned with resident goals, clinical needs, and facility requirements.
  • Participate in interdisciplinary team meetings, collaborating with nursing staff, rehab teams, social workers, and attending physicians.
  • Support clinical decision-making for transitions of care, including admissions, discharges, hospital transfers, and return-to-facility coordination.
  • Provide timely documentation in the electronic health record that meets regulatory and payor standards.
  • Educate residents and families on care plans, medication changes, health management, and disease progression.
  • Participate in quality assurance and performance improvement (QAPI) initiatives within the facility.
  • Serve as a clinical resource for SNF staff regarding resident care, clinical protocols, and best practices in geriatrics and post-acute care.
