About The Position

The Nurse Practitioner records physical findings, and formulates a plan and prognosis based on the patient's condition. This role involves discussing cases with physicians and other health professionals to prepare a comprehensive patient care plan. The Nurse Practitioner prescribes or recommends drugs or other forms of treatment, monitors and maintains the progress of patients, and reports this progress to the supervising physician. A key aspect of the position is the ability to work collaboratively with hospice employees and volunteers as part of an Interdisciplinary Team (IDT).

Requirements

• Minimum of (2) years experience in hospice, home health or community health in the past five (5) years.
• Completion of accredited Nurse Practitioner program.
• State licensure is also required.
• Ability to assist in lifting and transferring patients as required.
• Ability to lift objects in excess of 20 pounds.
• Specific vision abilities required by this job include: close vision, distance vision and ability to adjust focus.
• Ability to stand or walk in excess of one hour per day, talk, hear, sit, stoop, kneel and crouch as needed.
